This appointment is for the COVID-19 Vaccine.
Kaiser patients must go to Kaiser for their vaccines unless they want to self-pay.
We are not in network for Tri-Care (military benefits).
Choose ONLY MY CHILDREN OR DEPENDENTS to Schedule a COVID vaccine for your child if the parent/guardian is not also getting a vaccine at this appointment. Enter your (parent info) first, then click Add a person to this appointment to add your child and their info.
If the child has the same insurance as the parent, choose "Use Same Insurance" when prompted. If the child has different insurance (i.e. Medi-Cal), please un-check that box and enter the child's Medi-Cal number when prompted.
YOU MAY NOT CONSENT FOR YOURSELF UNLESS YOU ARE 18 YEARS OF AGE OR OLDER. YOUR PARENT OR GUARDIAN MUST COMPLETE THE CONSENT ONLINE AND THEY MUST EITHER BE PRESENT AT THE TIME OF VACCINATION OR PROVIDE THEIR SIGNATURE ON A PAPER CONSENT FORM. WE WILL NOT BE ABLE TO VACCINATE MINORS WITHOUT PARENTAL CONSENT. We appreciate your cooperation.
For patients receiving Comirnaty (Pfizer) 5 to 11 years old:
Single Dose of Comirnaty (Pfizer-BioNTech) COVID-19 2025-26 vaccine for ages 5-11 years or older recommended, regardless of previous vaccination status, at least 8 weeks after receipt of the most recent dose of any COVID vaccine.

For patients receiving Spikevax (Moderna) ages 6 months to 11 years old:
Ages 6 Months to 4 Years, Unvaccinated: Two Doses of Spikevax (Moderna) COVID-19 Vaccine, 4-8 weeks apart
Ages 6 Months to 4 Years, Previously vaccinated with ONE dose of Spikevax (Moderna): One Dose 4-8 weeks after last dose
Ages 6 Months to 4 Years, two or more doses of any previous Moderna vaccines, but not Spikevax (Moderna) 2025-26 version: One Dose at least 8 weeks after last dose
Ages 5 to 11 Years, Unvaccinated: One Dose of Spikevax (Moderna) COVID-19 Vaccine
Ages 5 to 11 Years, Previously vaccinated with one or more doses of Spikevax (Moderna): One Dose at least 8 weeks after last dosee
